Clarice Feldman at American Thinker explains it. He's actually conjoined twins separated at birth and that's the scar.

One twin, candidate Obama is charming, witty and a charismatic speaker. The other twin, President Obama, is charmless, witless and stutters badly without his teleprompter.

Clarice goes into a few other differences and it all makes sense to me.
